Jacob Sparks Avery

Jacob Sparks Avery, born February 10th 1991, the son of Jaymee and Aaron Avery, left this world to join his twin brother, Joshua, on Saturday, February 14th 2015 at Logan Regional Hospital. Jacob brought so much love, joy, and laughter to his family. Jacob left behind three brothers Kyle, Mitchell, and Kayson, and four sisters Tabetha, Passia, Shauna, and Jasmine. He is also survived by; his grandparents Dr. Barton and Ann Avery, Stephen and Judy Loftin, father Bill Mueller, uncles Robbie and Michael Loftin, Matt Bell, Eric, and Chad Avery, and aunts Bridgette Davis and Nancy Bell. Jacob loved laughing; his belly laugh and contagious smile will be forever remembered and cherished. A viewing will be held on Tuesday, February 17, 2015 from 6 – 8 pm at the LDS 15th Ward Church in Smithfield; 600 South 625 East, Smithfield. Funeral Service will be held at the Smithfield LDS 15th Ward Chapel on Wed. Feb. 18, 2015 at 11:00 am with a viewing from 9:30 – 10:30 am, at the same location in Smithfield.In lieu of flowers Jaymee and Aaron have requested that donations be made to the Make-A-Wish Foundation at Wishes at www.makeawishfoudation.org .
